Application Form

Thank you for your interest in St. Anthony Catholic School 

Applications for the 2025-2026 school year will open December 9th, 2024. 
 
Once your application is received we will reach out to schedule a tour. Tours will begin in Jan. 2025.
Applicants/students MUST attend a tour. 

If you are applying for multiple children please submit complete applications for each child.

  • Complete application checklist:
    • Completed Application
    • Baptismal Certificate (if applicable)
    • Preliminary Records Request form (please submit to your child's current school --1st - 8th grade) They will send the records we need to complete the application. Applications are not considered until records are received.
